Full instructions are listed on each individual form. All forms are PDF files, except for portal log-in links, and can be printed, filled out and returned to our Spoelhof location unless otherwise noted.

Directory information

Change of Address Form: Submit changes for local and home addresses through the portal. To update your on-campus address, you must contact the student life office.

Change of Name Form: If you are a current student or recent graduate, use this form to change your name.

Authorization to Withhold Directory Information: To withhold all directory information from the public, please submit this form.

Disability verification

The forms listed here can be printed, filled out and returned to our Hiemenga Hall location.

Acquired brain injuries/disabilities: To provide verification of your acquired brain injury/disability, use this form and attach any other reports, evaluations or test results if applicable.

Allergies: To provide verification of your allergies, use this form and attach any other reports, evaluations or test results if applicable.

Chronic health disabilities: To provide verification of your chronic health disability, use this form and attach any other reports, evaluations or test results if applicable.

Hard of hearing/deaf disabilities: To provide verification of your hard of hearing/deaf disability, use this form and attach any other reports, evaluations or test results if applicable.

Learning disabilities and/or attention deficit disorders: To provide verification of your learning disability, use this form and attach any other reports, evaluations or test results if applicable.

Low vision/blind disabilities: To provide verification of your low vision/blind disability, use this form and attach any other reports, evaluations or test results if applicable.

Mobility impairments/disabilities: To provide verification of your mobility impairment/disability, use this form and attach any other reports, evaluations or test results if applicable.

Psychological disabilities: To provide verification of your psychological disability, use this form and attach any other reports, evaluations or test results if applicable.

Graduation and commencement

Application for Degree: As a senior, you must submit this form via the portal to graduate, receive a degree, or participate in the commencement ceremony.

Program or advisor change

Change of Major and/or Advisor: You can use this form for major changes during your first year or for advisor changes any time.

Declaration of Major and/or Minor: You will need to submit this form during your sophomore or junior year. This is the final step in choosing a major and is required of all students.

Interdisciplinary Major Request: You can use this form to create a major from multiple departments.

Registration

Overload Request: If you want to take more than 17 credit hours (the maximum full-time load) during the regular semester, you must have advisor approval. Please return this form, signed, to academic services during registration. If your overload is approved, you will be billed for the additional credit hours.

Petition a Closed Class: Submit your petition for a closed course through the portal. You will not be able to petition core classes.

Schedule Builder: Use this chart to diagram your semester schedule or to help with registration planning.

Special course work and credit

Exemption or Credit Exam Application: If you know the material, you can test out of a course and receive an exemption, or class credit. You must take this form to the department offering the course to make arrangements and check deadlines.

Independent Study Request Form: Use this form to create a course that does not exist in the catalog with approval of an instructor.

Tutorial Request Form: Use this form to take a pre-existing catalog course on your own.

Student records

Verification Request: If you need a special letter to verify your registration, GPA or any other part of your Calvin record for your parent's insurance or for any other third party, use this on-line form to submit your request. Former students, sign and submit this release of records form for any of your verification needs.

Parent Verification Request: If you need verification for your son or daughter, use this form to submit your request.

Transcript Request Form: Visit our transcript request page to learn how to request official or unofficial Calvin transcripts.

Transfer

Transfer Credit Pre-Approval Form: Use this form to request approval for receiving Calvin credit for courses taken at another college prior to taking the course(s). For core and general elective credit, you may fill and submit the fillable pdf form here. Any major or minor equivalents must be approved by an advisor.

Michigan Uniform Undergraduate Guest Application: This form will serve as your application for admission as a guest student at a Michigan college or university. You should check with the other school to determine if there are additional requirements for admission. You will need to have your courses pre-approved to make sure they will transfer back to Calvin before you enroll at the other school.

Request for Leave of Absence (Hiatus): You may use this form to request an official leave of absence from Calvin College for a specific academic term. If you are approved, hiatus status will allow you to maintain access to Web mail, portal and Moodle accounts and you will not need to re-apply for admission upon return.

Incomplete Grade Contract: If you take an incomplete grade for a course, you should use this contract with the course instructor to document the terms of the completion agreement. Specifically, it should be used to outline the reasons for not completing the course by the set deadline and document the work necessary to complete the course.
