Generate a personalized information sheet

Instructions

1. In order to access your photograph, this must be done from a computer on Calvin's network.
2. Enter your Calvin userid below.

   Note: Your userid is the login you use to access Calvin email. It is probably something like fflint25. Do not use your alias if you have chosen one.

3. Click "OK".

   If you have entered your userid correctly, selecting OK below should present you with an information sheet to fill out. If your picture is available in the Calvin system and if you are on campus, it will appear printed on the information sheet.
