Selecting OK! will generate a form for you to fill out with your picture on it. Notes:

• You must do this from a computer located on campus for the picture to appear. Student pictures are not available from off-campus locations.
• If your picture does not appear on the form, be sure that you have correctly entered your Calvin user ID. If your ID is correct but your picture isn't showing, then there is probably some other sort of error and you may just continue. Else, go back and fix your ID and try again.
• If the form does not fit on one page, try adjusting the font size in your brouser. (In Netscape, for example, select "View | Increase Font" from the menu bar.)
