Hepatitis A

Schedule:
The Hep A vaccine series may be started whenever a person wishes to reduce their risk of the disease; or for travelers the vaccine series should be started at least one month prior to traveling for best protection. Two doses of the vaccine are needed for lasting protection. These doses should be given at least 6 months apart.		 Required for Entry to Calvin:

Hepatitis B

Schedule:
  • The first dose may be given anytime.
  • The second dose may be given 1-2 months after the first dose.
  • The third dose may be given 4-6 months after first dose.
  • The 0.5cc dose is given to individuals 19 and younger.
  • The 1cc dose is given to individuals 20 and older.

HPV-Gardasil

Schedule:
This vaccine is recommended for males and females between ages 19-26. This is a three shot series.The second and third doses should be given at two and six months (respectively) after the first dose.		 Required for Entry to Calvin:

Influenza/Flumist

Schedule:
All people 6 months of age and older should get the flu vaccine annually. It is available September through January.		 Required for Entry to Calvin:

MMR (Measles Mumps Rubella)

Schedule:
Children should get 2 doses of MMR vaccine: The first at 12-15 months of age and the second at 4-6 months of age. Adults who have not completed this series as a child should also get two doses of the MMR vaccine at least 4 weeks apart.		 Required for Entry to Calvin:

Menactra/Menveo (Meningitis vaccine) - Vaccine must cover strains A, C, Y, and W-135.

Schedule:
  • Give MCV4 to all unvaccinated teens, ages 13 through 18 yrs; if vaccinated at age 13-15 yrs, a booster dose at age 16-18 yrs is required.
  • Vaccinate all college freshmen who have not been vaccinated.
  • Give additional booster doses every 5 yrs if risk continues.

Polio

Schedule:
Adults who were not vaccinated as a child should get 3 doses of IPV. The first dose at any time, the second dose 1-2 months later, and the third dose 6-12 months after the second dose.		 Required for Entry to Calvin:

Tetanus Diphtheria

Schedule:
People who have not received at least 3 doses of any tetanus and diphtheria vaccine (DTP, Dtap, or DT) during their lifetime should do so. After a person receives the third dose, a Td booster is needed every 10 years.		 Required for Entry to Calvin:

Tetanus Diphtheria Pertussis

Schedule:

Tuberculin Testing

Schedule:
People who answer "Yes" to any questions on the "TB Self-screening form" will need to get a TB test. Also, people who have traveled outside of North America will need a TB test 3 months after their travels. The TB test will need to be read within 48-72 hours after administration. Typhoid (Injectable or oral)

Schedule:
Inactivated vaccine given at least two weeks prior to traveling to areas of risk. A booster is needed every two years. Live (oral), 4 doses given at least 1 week before travel abroad. Booster needed every 5 years.		 Required for Entry to Calvin:

Varicella

Schedule:
Those who have never had chicken pox disease should receive 2 doses of the vaccine. First dose, 12-15 months of age, 2nd dose 4-6 years old, but at least three months after the first dose. People 13 years or older should get 2 doses, 28 days apart or more. 		Required for Entry to Calvin:

Yellow Fever

Schedule:
Persons 9 months of age or older traveling to or living in a country that requires yellow fever vaccination should get this vaccine. Persons 9 months of age or older traveling to or living in a country that does not require yellow fever vaccination but is located in an area where the risk of yellow fever is known to exist should get this vaccine. Re-vaccinate every 10 years if continued risk exists. 		Required for Entry to Calvin:
