Supplemental insurance for overseas directors
Since you will be out of the country beyond the 90-day limit imposed by Priority Health, Calvin will purchase a "gap" policy to cover you from day 91 to the end of your program. (If you choose to stay more than a week beyond the end-date of the program, you will be responsible for the cost of the portion of the supplemental insurance attributable to your extra days away.)

- Complete the Enrollee Information – Diplomat International form except for the "Payment Method" section and submit it to Off-Campus Programs. OCP will handle the payment of the premium. (It is helpful if you put "Buiten & Associates" in the blank for the agency. No agent's name is needed.)
