Travel Health Online Module
Whether you are planning an adventure abroad for fun, study, or mission work, our goal is to keep you healthy and safe. This online travel module has been designed to help you prepare for your trip abroad.
Please review the online module carefully. At the end of the presentation, you will be asked to print off a .pdf document. Please complete the document and turn it in to Health Services as soon as possible. You will be required to make an appointment to review your immunizations and travel requirements at that time. Please note we would like all travelers to be complete with their travel requirements prior to November 30th.

Fees and payment
If you are traveling with a Calvin-sponsored program, many of the required vaccinations are included in your course cost. If you need additional vaccinations for your group travel, or if you are an individual traveler, you will need to pay for your vaccinations on the day of your appointment.
