Your patient rights and responsibilities
As a recipient of health care services at Calvin, you have choices, rights, and responsibilities. We want to provide you with the very best health care and encourage you to be an active and informed participant.
You have the right to:
- Be treated with dignity and respect in a safe environment
- Personal privacy
- Confidentiality of all personal, medical, and financial information
- An explanation of your care and treatment
- Participate in decisions about your care
- Know about the availability and cost of services
- Know the names/positions of professionals serving you
- Review your medical records
- Request to change health care providers, seek services elsewhere, or obtain a second opinion
- Provide us with feedback about our services
You have the responsibility to:
- Provide accurate and complete information about all matters pertaining to your health
- Report any changes in your health to us, including new symptoms or pain
- Make it known if you do not understand the treatment and services offered
- Participate in planning and carrying out your treatment plan
- Learn about your medications, accepting responsibility for taking them as prescribed or properly disposing of them (if not used)
- To fulfill your financial responsibilities
- Keep scheduled appointments or cancel them in advance if they are no longer needed
We feel you should be aware that in the State of Michigan, it is a felony to create, manufacture, or distribute a controlled substance. It is also a felony to create, possess, or deliver a prescription form except as provided by a licensed professional. Either offense is punishable by imprisonment or substantial fines or both.
