You Have the Right to:

  • An explanation of your care and treatment
  • Participate in decisions about your care
  • Know about the availability and cost of services
  • Know the names/positions of professionals serving you
  • Review your medical records
  • Request to change health care providers (if available)
  • Provide us with feedback to improve our services
You Have The Responsibility To:
  • Be honest about your medical history
  • Report any changes in your health
  • Make it known if you do not understand the treatment and services offered
  • Participate in planning and carrying out your treatment plan
  • Know your medications by name and purpose
  • To fulfill your financial responsibilities
  • Make/keep/cancel your appointment

More About Your Confidentiality:

All records are private and confidential. Medical records are not released without the student's signed consent except in an emergent situation.

There are a few confidentiality exceptions required by law (such as reporting certain communicable diseases) or there may be situations that threaten your own safety or the safety of others. If you are under the age of 18, additional regulations apply.

You should be aware of your rights under the Family Educational Rights and Privacy Act (FERPA) and The Health Insurance Portability and Accountability Act (HIPAA) .
