2008 SUMMER SERVICES FOR KNIGHTCARE STUDENTS

During the summer months Health Services limits medical services to students who are enrolled in the KnightCare student insurance plan.

Beginning May 21, appointments are available on Tuesdays and Thursdays from 1-4 PM. Call (616) 526-6187 (option 1).

Additional Information
Services: Generally, reduced summer hours will not permit appointments for preventative or maintenance exams such as health physicals, annual exams, gynecologic exams, and others. Should the need arise, call us for area referrals.

We apologize for any inconvenience our reduced summer hours may cause and hope that advance planning will help to ease any problems. Don't hesitate to call us if you need additional assistance to meet a particular health problem over the summer.

Graduating? Remember that Heath Services is available only to currently registered students. Ask us about area referrals.

Immunization Records? If you'd like an up-to-date copy of your immunization record for graduate school, employment requirements, or just to keep on file, email us at health@calvin.edu. Be sure to include:
Your full name
Your student ID number
Your current address
Your phone number
You may also stop by the Immunization and Insurance Office at Health Services to pick up a copy.
