Calvin requires all new students to complete a health history form and to show proof of their immunization and health insurance status. You are NOT required to have a physical exam, although a health care provider's signature is required on your immunization history form.

You can access the required forms either online (through e-Enroll) or by printing the forms (see the links below) and returning them to us by fax or mail. Forms must be completed and returned to Health Services by October 1st . Our health form fax number is 616-526-6548.

All students will be enrolled in the college-sponsored Student Health Insurance Plan (KnightCare) unless they show proof of existing insurance and submit a signed waiver.

Please check your insurance coverage carefully before waiving out of the KnightCare insurance plan. Some things to keep in mind: 1) Travel or Emergency-Only plans will not be accepted as comparable coverage. Examples of travel plans are Blue Cross Blue Shield Travel Plan, Student Blue, TIC, and CAA; 2) Many out-of-state plans will consider physicians and hospitals in the Grand Rapids area as out-of-network. Check to see what your coverage will be for providers nearest to Calvin College.
