Insurance Forms
The American College Health Association recommends that all college students have health insurance as a condition of enrollment. Calvin College recognizes that students may encounter unexpected health problems and need access to medical care while studying at the college. For this reason we require students taking more than 6 credit hours to have health insurance. Students will be enrolled in the college-sponsored plan (KnightCare) unless they provide proof of comparable insurance and submit a signed waiver.

Enrolling in KnightCare
It is easy to enroll in KnightCare. Simply fill out an enrollment form or contact our Immunizations and Insurance staff at 616-526-6568. The KnightCare premium will be billed to the student's miscellaneous account, and payment can be made through Financial Services.

Waiving out of KnightCare
If you choose to waive out of KnightCare, you must be prepared to answer questions about your current health insurance coverage. Students might need to ask their parents for some help with this if they are covered under the parent's plan.

This is your chance to enroll in or waive out of Calvin's student health insurance plan for the next school year. If you choose to waive out of KnightCare, you must be prepared to answer questions about your current health insurance coverage.

Insurance information must be submitted by April 15 or your account will be billed for KnightCare in May.
