| What is KnightCare? | |||||||||||||||
| KnightCare is a student health insurance plan was developed specifically for the students of Calvin College. The plan is offered through StudentResources, a United HealthCare Insurance Company. The plan pays the costs of medical services for covered illnesses and accidents up to $50,000 per policy year. | |||||||||||||||

| Who is eligible? | |||||||||||||||
| All students taking more than 6 credit hours are automatically enrolled in and charged for the KnightCare unless proof of comparable coverage is provided. Please review Calvin's insurance requirements for more information. | |||||||||||||||
| Students enrolled in KnightCare who graduate in May are still covered under the plan until August 15th of the policy plan year. Coverage may also be purchased for spouses and dependent child(ren) under age 19. | |||||||||||||||
| How much does KnightCare cost? | |||||||||||||||

| *Enrollees will also be charged a $50.00 administrative fee. Those who pay in full by May 31 (premium and administrative fee) will receive a $25.00 early payment administrative fee discount. | |||||||||||||||
| When will I be billed? | |||||||||||||||
| If you are an incoming student, you can expect to see the KnightCare premium on your miscellaneous account in August. For all returning students, the charge will be placed on your account in May. | |||||||||||||||
| What is my deductible? | |||||||||||||||
| KnightCare has a one-time $100.00 deductible per policy year for medical services received off-campus. It is common to receive this charge the first time you use off-campus services. | |||||||||||||||
| Does the Student Health Insurance cover prescription drugs? | |||||||||||||||
| KnightCare will pay up to $1000.00 for prescription drugs at area pharmacies per policy year. There is a $15.00 copay for generic drugs, and a $25.00 copay for brand name drugs. | |||||||||||||||
| On campus, KnightCare will pay up to $1000.00 for prescription drugs per policy year. There is a $5.00 copay for generic drugs, and a $10.00 co-pay for brand name drugs. Both copays cover a one-month supply. Over-the-counter medications are not covered. | |||||||||||||||
| Can I keep my parents' plan and sign up for KnightCare? | |||||||||||||||
| Yes. Many families decide to supplement their current policy with the Student Health Insurance Plan, especially if their HMO reduces benefits while the insured is out of area. KnightCare will be your secondary insurance. When filing a claim, simply submit the claim and associated receipts to your family insurance. After the primary insurance has made payment, submit the claim to StudentResources for the excess amount (for any covered accident or illness). | |||||||||||||||
| What benefits are covered at Calvin Health Services? | |||||||||||||||
| KnightCare will pay unlimited benefits for covered services at the Health Center (including visit costs and lab tests). There is no copay for the visit, and the student will not be billed. These services are provided for current students only. A referral list of area providers is available for spouses and dependents. | |||||||||||||||
| Medications dispensed from the Health Center are covered at 100% up to $1000.00. You will be charged a copay for prescriptions: $10.00 for brand-name drugs, and $5.00 for generic drugs. Over-the-counter medications are not covered at the Health Center, but can still be purchased if recommended by one of our providers. | |||||||||||||||
| Are benefits for counseling available? | |||||||||||||||
| Yes. KnightCare covers 80% of off-campus costs up to a maximum of $1000.00 per year. On-campus visits to Broene Counseling Center are free-of-charge. Psychiatric and attention-deficit evaluation and treatment are available on a part-time basis. If you are seen by one of these on-campus Calvin/UnitedHealthcare-contracted providers, you will receive 100% coverage up to $300.00, and then 80% up to $1000.00. | |||||||||||||||
| What happens if I have to go to an off-campus doctor? | |||||||||||||||
| Covered off-campus medical office visits are payable at $65.00 per visit, up to 10 visits per illness, per policy year. The insured is charged a $10.00 copay, and is responsible for 20% of the cost of the visit after your deductible has been met. To maximize your savings and limit your out-of-pocket expenses, select a preferred provider from UnitedHealthcare's extensive PPO list. Health Services can supply you with a list of area providers who participate with UnitedHealthcare. | |||||||||||||||
| Does the plan include dental coverage? Vision coverage? | |||||||||||||||
| No, there is no coverage for dental or vision. | |||||||||||||||
| Can I go to the Emergency Room? | |||||||||||||||
| Yes, you may receive benefits for the emergency room. However, there must be a diagnosed sickness or accident, and it must be considered an emergency (life threatening). Benefits for covered illnesses and accidents are payable up to a maximum of $5000.00 per diagnosis. | |||||||||||||||
| If you need to be hospitalized, KnightCare will pay up to $400.00 per day for Room & Board. Other benefits are payable for surgeries, anesthetist, etc. | |||||||||||||||
| Are tests like a CAT Scan or MRI covered? | |||||||||||||||
| Yes, diagnostic x-rays, tests, procedures and laboratory services are covered under the Outpatient Miscellaneous benefit. These tests are covered at 80% up to a maximum of $10,000.00 for covered illnesses and injuries for preferred providers. | |||||||||||||||
| Will KnightCare cover me while I am abroad? | |||||||||||||||
Yes. KnightCare provides worldwide coverage. Medical expenses will be paid according to the out-of-network benefit. |

| I'm getting bills in the mail. What do I do? | |||||||||||||||
You may be receiving bills for your deductible. Remember, you are responsible for the first $100.00 of medical expenses before KnightCare begins paying. | Unfortunately, KnightCare does not extend it's coverage past August 15th of the policy year. If you are graduating and need insurance options, look at our Temporary Insurance page for more information. | |||||||||||||||
