The Student Health Insurance Plan (KnightCare) offered through Calvin College is a comprehensive health insurance plan designed with a student's needs and budget in mind. Most important, KnightCare gives students access to an extensive network of health care providers practicing in the Grand Rapids area, as well as nation-wide.

When Coverage Begins
Spring coverage starts at 12:01 am on January 1, 2008, and continues through 12:01 am on August 15, 2008. KnightCare Premium for Spring coverage is $585.00.

Annual coverage for the 2008-09 academic year will begin at 12:01 am on August 15, 2008, and continues through 12:01 am on August 15, 2009. KnightCare Premium for Annual Coverage is $973.00.

 

KnightCare Features and Benefits
One major benefit of the KnightCare student health insurance plan is that visits to the Health Center on campus are completely covered. We strongly encourage students with KnightCare to visit Health Services first for their medical needs. There are no visit co-pays or deductibles to worry about here.

The chart below shows how KnightCare pays benefits for the major types of health care expenses off-campus. You can see the difference in your cost when you use preferred providers.

*All Outpatient Mental Health is subject to an aggregate maximum benefit of $1,000 following a $10 co-pay per visit.
**Prescription Drugs are covered with a $15 per prescription co-pay for generic prescription drugs and a $25 per prescription co-pay for brand-name prescription drugs up to a $1,000 Policy Year maximum.

Getting Enrolled
It is easy to enroll in the Student Health Insurance Plan. Simply fill out an enrollment form or contact our Immunizations and Insurance staff at (616) 526-6568. Coverage may also be purchased for spouses and dependent child(ren) under age 19.

 
Filing Claims
On-Campus: Charges for visits to Health Services will be billed directly to the Student Health Insurance Plan. You will receive no bill for services received on campus, however there is a $1,000 limit on prescriptions.
Off-Campus: You need to carry your insurance card at all times. Most health care providers will file a claim for you. If a provider does not file a claim on your behalf, you will need to pay cash. It is your responsibility to initiate a claim for reimbursement.
