Calvin College requires all incoming students to submit immunization information to Health Services. Documentation showing your immunization history or proof of immunity (e.g. dates of vaccinations or positive antibody titers), needs to be submitted to Health Services prior to your arrival on campus. |

Required Immunizations | |||
| You probably already have received these required immunizations. To obtain vaccination records, try contacting (1) your family physician, (2) your high school or previous college, (3) your branch of military service. | |||
| To submit your records to us, you can have a physician complete and sign our Immunization History Form. Or, if you have an official record of your immunizations, you may send us a photocopy. | |||
| If you would like to have your vaccines or blood titer completed at Health Services once you arrive on campus, please call us at (616) 526-6568 to schedule an appointment. For a complete listing of the vaccinations we administer, and their prices, please view our encounter form. | |||
