Submit your immunization history/get immunizations
As a new student at Calvin College, you are required to submit your immunization history to health services.
You must submit documentation showing your immunization history, with dates of vaccinations or positive antibody titers, before you arrive on campus.
How to submit your immunization history
You've probably already received all required immunizations. You can track them down through:
- your family physician
- your high school or previous college
- your branch of military service
To submit your immunization history, you can:
- have your physician complete and sign an immunization history form (pdf)
- or send us a copy of your official record
Complete your immunizations
If you would like to have your vaccines or blood titer completed at health services once you arrive on campus, please call us at (616) 526-6187 to schedule an appointment. For a complete listing of the vaccinations we administer, and their prices, please view our immunizations list.
Deadlines
For all new, transfer and re-admitted students, your health requirements must be complete by October 1st. After that time, a non-refundable $50 fine will be assessed to your account, and your registration could be placed on hold.
Tuberculosis screening
Before you start classes at Calvin, complete a tuberculosis self-screening using our downloadable form. If you fall into a high-risk group for tuberculosis, you must have a tuberculosis test completed at health services upon arrival.
