By giving us your feedback, we'' be able to continue to give you our best.

You may take each sturvey one time only. The surveys will be live from Monday, November 3 until 11:59pm on Thursday, November 6. We will be drawing from all surveys received to award the winner an Ipod NANO on Friday, November 7. When you complete the survey, you must send an email to jw25@calvin.edu and include your name, telephone number, and the secret code word that appeared on the last page of the survey. Good Luck!
