Using the Empowerment Model to Bring Health Ministry to the Homeless Population

This presentation describes the empowerment process used to develop a health ministry team at a store front church in an urban neighborhood with a high homeless population. A community nursing model that incorporates the concept of empowerment was employed to work with a group of homeless adults in assessing the needs of those served by the faith community. The health ministry team used the model to develop a plan to address a priority health issue, nutrition. The process utilized resources from a variety of providers within the homeless neighborhood and broader community to improve the health of the homeless community. The application to other settings, description of the health intervention implemented, outcomes, challenges and recommendations will be discussed.
